Fluency and coherence
Hesitation is mostly content-related — you pause to think about the idea, not to find the word — and the pauses fall at clause boundaries, where a listener expects them. You can develop an answer without being prompted.
Pronunciation
Word stress is usually right, sentence rhythm carries the meaning, and the difficult sounds are right more often than not. What remains is inconsistency: the sound you produce perfectly in a careful sentence slips when you speak quickly.
